This article explains how you can create a new form using the form builder in the settings module.


Manage Forms Builder

  1. Navigate to the Settings tab and locate the "Forms Builder" option under Facility Setup.
  2. All the forms created by the admin for the facility will be available in this screen with the ability to view, edit, copy, and delete the forms.
  3. On the right top corner of the screen, the "+Create New Form" button is used to create a form for the facility.
  4. Action Options - 
    1. View Icon - Click this icon to view the form in a new tab.
    2. Edit Icon - Click this icon to edit the form in a new tab.
    3. Delete Icon - Click this icon to remove the form.
    4. Copy Icon - Click this icon to create a copy of the selected form.
