This article explains how to create a new form in the Form Builder.


How To Create A New Form in Form Builder?


  1. Click the Settings tab in the left navigation.
  2. Select the Form Builder option under Facility Setup.
  3. Click on the orange "+Create New Form" button in the top right. It will redirect you to the new screen. 
  4. The Create New Form screen allows you to create a custom form.
    1. Enter form title: Enter the title of the form.
    2. Select form type: Use the drop-down list to select the type of treatment for the form.
    3. Add fields to the form: This section has numerous types of fields - text field, selection field, header, file upload, and so on. Select the required field. It will appear on the screen. Consecutively, you can also left click to drag (the cursor will turn into a plus icon) the field to the form area.
    4. Form area: Drag and drop all the necessary fields in this area. This will become a new form once you save changes. 
    5. Edit functions: To edit the fields in the form area, place your mouse cursor in the field area. The edit function buttons will appear on the right top corner of the respective field.
      1. Click on the "X" icon to remove the field.
      2. Click on the "pencil" icon to input the values. 
      3. Click on the "papers" icon to copy the field and duplicate it. 
      4. Click on the "upward arrow" to move the field above.
      5. Click on the "downward arrow" to bring the field below.  
      6. Once you have entered the required values, click on "Close".
      7. After creating the form, click the Save button to create the form.
