How To Create A New Form in Form Builder


  1. Click the Setting tab in the left navigation.
  2. Select the Form Builder option under Facility Setup.
  3. In the Form Builder screen, click the +Create New Form button.
  4. The Create New Form screen opens where users can create a custom form
    1. Enter form title - Enter the title of the form.
    2. Select form type - Use the drop-down list to select the type of treatment the form will be associated.
    3. Add fields to the form - This section has numerous types of fields like text field, selection field, header, file upload, and so on. Click the field needed and hold on to your left mouse click to drag (the cursor will turn into a plus icon) the field to the form area to the left.
    4. Form area - Drag and drop all the necessary fields in this area which will then become the form after clicking the Save button.
    5. Edit functions - To edit the dropped fields in the form area, place your mouse cursor in the field area to make the edit function on the right top corner of the field.
      1. Click on the "X" icon to remove the field.
      2. Click on the "pencil" icon to input the values. 
      3. Click on the "papers" icon to copy the field and duplicate it. 
      4. Click on the "upward arrow" to move the field above.
      5. Click on the "downward arrow" to bring the field below.  
      6. Once you have entered the required values, click on "Close".
      7. After constructing the form, click the Save button to create the form.
