This article explains how to create a new form using the form builder in the settings module.

TherapyPM allows you to create and customize your own forms with the help of a tool "Form Builder" in the Settings module. 



How to Manage Forms Builder?

  1. Navigate to the Settings tab on the left. Locate the "Forms Builder" option under Facility Setup.
  2. The forms that you created will be accessible in the list. You can view, edit, copy, and delete them as required. 


How to Create a New Form?

  1. Click on the orange "+Create New Form" button on the top right corner to create a form for the facility. For more information on creating the new form, visit this article. 
  2. For further actions, you can navigate the Action column (right):
    1. View Icon: Click the eye icon to view the form in a new tab.
    2. Edit Icon: Click the paper-pen icon to edit the form in a new tab.
    3. Delete Icon: Click the red trash icon to remove the form.
    4. Copy Icon: Click the paper icon to create a copy of the selected form.


Available Field Types and Their Purpose


Field TypeDescription
AutocompleteAllows users to search from a list of predefined values.
ButtonAdds a clickable button; useful for submitting or navigating forms.
Checkbox GroupDisplays multiple options with checkboxes. Users can select more than one.
Date FieldFor selecting a date using a calendar picker.
File UploadEnables clients or staff to upload documents or images.
HeaderAdds a title or heading text to separate sections.
Hidden InputA non-visible field that stores preset values for internal use.
NumberAccepts numeric input only. Useful for quantities or scores.
ParagraphAdds text for instructions or descriptions. Non-interactive.
Radio GroupDisplays a list of options where only one can be selected.
SelectA drop-down menu for single-choice selection.
Text FieldShort, single-line input for names or titles.
Text AreaMulti-line text input for comments, feedback, or narratives.



Field Action Icons


Each field added to the form includes four action icons in the top-right corner:

EditOpens settings to change the label, make it required, or adjust details.
DeleteRemoves the field from the form. This action cannot be undone.
⬆️Move UpMoves the field one position upward in the form layout.
⬇️Move DownMoves the field one position downward in the form layout.



Tips for Effective Form Building

  • Use Header and Paragraph fields to clearly section and label your form.

  • Place required fields thoughtfully to improve data collection.

  • When setting up options for fields like checkboxes, radio groups, or select menus (e.g., “Yes,” “No,” or multi-select choices), ensure that both the label and value fields are filled in. This guarantees that selections save and display correctly. 

  • Click Save regularly to prevent data loss.
