To delete a contract rate row:

1. Navigate to Billing > Contract Row.
2. Select the insurance and treatment to generate a contract rate table. 
3. Under the Actions column, click on the three horizontal dots (...) under selected rows and click Delete.

The contract rate table will now be removed.
