This article explains how the staff can upload intake forms.


How To Upload Intake Forms: 


  1. Click the Intake tab in the Provider Bio module
    • All the intake forms assigned to the provider will be available in the Intake section
  2. Click the file name in the table to download the intake form.
    • Fill in the required information and complete the intake form.
  3. Click the Upload icon under the Action column to upload the completed intake form.
  4. The status of the intake form submission can be viewed under the Submitted column.
    1. If the icon shows "✓", the form is submitted.
    2. If the icon shows "X", the form is not submitted.
  5. Status - Review the status of the form, which includes "Completed," "Pending," and "Needs Revision."
  6. Comments - View the comments left by the admin by clicking on the message icon. If there is a comment, the icon will be highlighted in green.
