This article explains how the staff can upload intake forms


How To Upload Intake Forms:


A.Click on "Biographics" on the dashboard menu and click on "Intake". 


B. Find the intake forms assigned to you and complete them by following these steps. 

  1. Download - Click on the file name to download the intake form
  2. Fill in the required information and complete the form. 
  3. Upload - Click on the upload icon, located under the actions columns, and upload the completed forms to the software.


C. Further Actions:

  1. Submitted  - You can easily track your submission status by checking the "✓" for submitted forms and "x" for forms not yet submitted, under the Submitted Column.
  2. Status - Review the status of the form, which includes "Completed," "Pending," and "Needs Revision."
  3. Comments - View the comments left by the admin by clicking on the message icon. If there is a comment, the icon will be highlighted in green.
