This article details on uploading intake forms for the facility and managing the forms.


Navigate to the Settings tab and click on Intake Form underneath Facility Setup


How to manage intake forms:

  1. All the intake forms available in the system will be in the Intake Form data table.
  2. The status of the intake forms can be made active or inactive.
  3. The action column provides the below features
    1. Download - Click the icon to download the form.
    2. Delete - Click the icon to delete the intake form.
  4. Add Intake Form - Click the button to add a new intake form. 


How to add a new intake form:

  1. Click the Add Intake Form button.
  2. For: Choose if it is for employee or client
  3. Add the document.
  4. Click the Upload button
