This article explains how to upload intake forms for the facility and manage them effectively.


How to manage intake forms?

Navigate to the Settings tab (left). Go to the Facility Setup and click on Intake FormsNote: All available intake forms are listed in the Intake Forms list. 

  1. The status of the intake forms can be made active (toggle ON) or inactive (toggle OFF).
  2. The Action column (right) provides the below features:
    1. Download: Click the down arrow icon to download the form.
    2. Delete: Click the red trash icon to delete the intake form.


How to add a new intake form?

  1. Click the Add Intake Form button in the top right corner. A pop-up will appear, as shown below. 
  2. Choose whether the form is for an employee or a patient, in the drop-down under "For:".  
  3. Choose the desired file under "Browse Intake Form".
  4. Click the Upload button.



Important: Upload-Based vs. Template-Based Intake Forms

TherapyPMS supports two types of intake forms:

  • Upload-Based: Upload your own custom PDF or document into the system.

  • Template-Based: Create forms using the built-in Form Builder.

The system automatically locks your account to one of these formats based on the method used when adding your first intake form.

Key Rules:

  • If your first form is uploaded, your account will be restricted to upload-based intake.

  • If your first form is built using the template tool, your account will be restricted to template-based intake.

  • Switching between formats (upload ↔ template) is not allowed through the user interface after this initial setup.




Need to Change Intake Type?

If you need to change your intake format, please contact TherapyPM Support. We can assist with switching, but please be aware:

⚠️ Warning: Switching intake types will permanently delete all existing intake data submitted by clients, including the “Date Submitted by Client” field.
