Exclude patients from being incorrectly selected for scheduling with certain providers

1. Patient - Select a name from the list, or select multiple names with either the Ctrl key or Shift Key (while left-clicking).
2. Exclude Selected Patients - Excludes all the selected patients and moves them to the list on the right.
3. Remove - Remove previously excluded patients from the list.
