Prevent service levels from being scheduled for specific staff

1. Service  - List of all current services in the practice, One can be clicked/selected or Ctrl key can be used to select more than one.
2. Exclude Selected Service(s) - Excludes all of the selected services on the left side and places them on the right side
3. Delete - Removes an exclusion from the staff member
